Dr. Joseph Freeman is a board certified Family Medicine Physician and is also a native of Covington, TN. He trained locally at the University of Tennessee College of Medicine. He is committed to providing the high quality healthcare that this community deserves.
Patient Payment Information
Co-Pay
Co-pays are expected to be paid at time services are rendered. You may receive a statement from the clinic showing the balance you owe for your visit after insurance has covered it's part of the payment.
High Deductible Health Plan
If your insurance is a High Deductible Health Plan, payment for your visit is expected at the time services are rendered. The fee for a standard office visit is $200. This fee does not include injections, in-house labs, labs that are sent off for testing, or any procedures. You will be billed for any extra costs after your claim is submitted. Once your deductible is met, your insurance will cover the percentage listed in your plan.
Self-Pay Program
If you are uninsured or choose to pay out of pocket for your visit, you will be expected to pay for your visit at the time services are rendered.
Initial Visit/Establish Care: $150
Follow-up Visits: $100
Sick/Urgent Visit: $100
This fee does not include in-house labs, labs that are sent off for testing, injections, or procedures.
*If you have health insurance and choose the self-pay option, your insurance will not be run, and payment will not go towards your deductible.
Payment Plan
We offer payment plans if you receive a bill and would like to break it into monthly payments until your balance is met. If you are interested in a payment plan, ask the front desk for more information, and they can help get it set up.
